Advantages Of The Quantiferon Tb Gold Include:
- Not needing to return for the test results like the TB skin test which requires 2 visits
- BCG vaccine will not cause a false positive in Quantiferon test as it sometimes does with a TB Skin test
- Results are usually available in 3-5 business days
- Can be used in all circumstances in which a TB Skin test is recommended
Who Should Not Take The Quantiferon
Besides children, there are certain population groups that are not considered viable candidates for taking this blood test. Anyone who has an altered immune function fits into this category. This may be the result of an autoimmune disorder, an HIV infection, or people who are receiving immunosuppresive medications.
Certain clinical conditions may also alter the effectiveness of the results. This includes people who have a confirmed diagnosis of diabetes, renal failure, certain cancers and blood disorders, and silicosis.
Women who are pregnant are also not considered good candidates for this blood test.
The CDC, however, has approved the use of this blood test as a screening tool for virtually all individuals based on current data, even if the results from the test have not been studied. This is why it is important to speak with your provider about this specific blood test to see if it would be right for you.
How Much Does A Quantiferon Tb Gold Test Cost
The cost of a Quantiferon TB Gold blood test in a laboratory setting will cost about $90 to $160 without a health insurance plan. The costs will depend on the lab you choose and if health insurance is involved.
Accesalabs.com, for example, charges $149 for its test.
WalkinLab.com, another lab online, will charge $148. The laboratory does note that a special collection kit will be required and may not be available at all LabCorp locations. This special collection will contain three gel-barrier tubes.

Question 4 How Do You Interpret The Quantiferon
An example of a QFT-Plus report is shown below:
Nil or Negative Control Tube
The nil value comes from the negative control tube, which contains no additives it is used to determine if the patient has a preexisting immune response , which could cause a false-positive reading on the test.
For a test to be valid, the nil tube must have a value of 8.0 IU/mL. In the example cited above, the nil value is well within the recommended range of 8.0 IU/mL. The nil tube value is subtracted from the values of the mitogen, TB1, and TB2 tubes for the final result of the respective tubes.
Mitogen or Positive Control Tube
The mitogen, or positive control, tube contains a mitogen , which is a nonspecific stimulator of T-cells. It is used to assure that the patients T cells are capable of producing IFN- and also serves as a control for correct blood handling and incubation. The mitogen tube is also used to detect false-negative readings.
For a test to be valid, the mitogen tube must have an IFN- value at least 0.5 IU/mL higher than the value of the nil tube. This indicates that the test is functioning properly.
In the example cited above, the mitogen tube value minus the nil tube value is well above the minimum breakpoint of 0.5 IU/mL.
The inside of this tube is coated with M tuberculosisspecific antigens designed to elicit CMI responses from CD4+ T-helper lymphocytes.

Quantiferon Tb Gold Test Positive Treatment
The quantiFERON TB gold Test positive treatment includes the following steps
- Special medication prescribed for the period of 6 to 9 months.
- Use of gamma interferon therapy to monitor the treatment of tuberculosis.
- The treatment is a long procedure owing to the slow growth of the M. Tuberculosis bacteria and also takes a lot of time to kill the bacteria.
- Doctors can change the medication in course of the treatment in response to the emergence of drug resistant organisms.

Additional Considerations And Recommendations In The Use Of Qft
QFT-G can be used in all circumstances in which the TST is used, including contact investigations, evaluation of recent immigrants who have had BCG vaccination, and TB screening of health-care workers and others undergoing serial evaluation for M. tuberculosis infection. QFT-G usually can be used in place of the TST.
A positive QFT-G result should prompt the same public health and medical interventions as a positive TST result. No reason exists to follow a positive QFT-G result with a TST. Persons who have a positive QFT-G result, regardless of symptoms or signs, should be evaluated for TB disease before LTBI is diagnosed. At a minimum, a chest radiograph should be examined for abnormalities consistent with TB disease. Additional medical evaluation would depend on clinical judgment on the basis of findings from history , physical examination, and chest radiography. HIV counseling, testing, and referral is recommended because HIV infection increases the suspicion for TB and the urgency of treating LTBI. After TB has been excluded, treatment of LTBI should be considered .
What Is Tb Quantiferon Test
TB Quantiferon is a special blood test used to detect the presence of Mycobacterium tuberculosis infection. It replaced the century-old TB skin test which is also known as PPD, Mantoux test, tuberculin skin test or TST.
What is a TB Skin Test?
Before, the only way of detecting Mycobacterium tuberculosis infection is to do a TB skin test. Its done by injecting a small amount of TB protein under the skin. If you have an active TB infection or if you have been exposed to Mycobacterium tuberculosis, a wide red bump will develop around the injection site.
How does a TB test look like when it is positive?
When a tuberculin skin test is positive, an induration or localized swelling will appear after 1-3 days. It has to measure more than 5mm in diameter to consider it positive.
What are the advantages and disadvantages of a tuberculin skin test vs the Quantiferon-TB test?
A tuberculin skin test is cheaper than the QuantiFeron-TB Gold Plus. However, it cannot differentiate active or latent infection and it takes three days to be read.
The QuantiFeron-TB Gold Plus, on the other hand, can confirm active tuberculosis infection. It uses highly specific TB antigens that aid in properly detecting the presence of TB infection in the blood. It takes a maximum of 24 hours to get the results.

Reminders On How To Do Qft
- If you are using a butterfly needle, use a discard tube before the QFT-Plus tubes to remove the air along the butterfly tubing.
- Remember that QFT-Plus tubes are vacuum-pressured to collect exactly 1mL of blood in each tube. The blood will stop filling the tube once the tube collected 1mL already. Thats why its very important that there is no air present in the butterfly tubing before attaching the QFT-Plus tubes for blood collection.
- QFT-Plus tubes fill up very slowly. This might make you think that you hit a bad vein but be assured that the tubes are made to collect blood slowly.
- After filling the tubes, shake them 10 times to ensure that the surface is entirely coated by blood. Over or under shaking will produce erroneous results.
- The tubes should be delivered upright to the laboratory.
- At the time of collection, the tube temperature should be between 17-25 degree Celsius.
- Some healthcare institutions may require special training before a nurse or a phlebotomist can do a QFT-Plus test.

QuantiFERON-TB Gold Plus is an in vitro diagnostic test using a peptide cocktail simulating ESAT-6 and CFP-10 proteins to stimulate cells in heparinized whole blood. Detection of interferon- by enzyme-linked immunosorbent assay is used to identify in vitro responses to these peptide antigens that are associated with Mycobacterium tuberculosis infection. QFT-Plus is an indirect test for M. tuberculosis infection and is intended for use in conjunction with risk assessment, radiography, and other medical and diagnostic evaluations. This test does not differentiate between latent and active TB. Up-to-date product-specific warnings and other information can be found at www.QuantiFERON.com.

Tb Blood Test Vs Tb Skin Testing
Traditionally, TB screening has been done through a TB skin test, also known as a PPD test or a Mantoux test. To get a TB skin test, one needs to visit a medical office and receive a small injection of a substance known as a protein purified derivative in the forearm. After 48-96 hours, the forearm needs to be checked and measured by a medical provider to see if any swelling, or induration, has developed at the site of the injection. If a raised swelling above a certain size is noted on the second visit, a chest x-ray is required to make sure that tuberculosis is not present in the lungs. As a result, the TB skin test requires a minimum of two visits and possibly more if the TB skin test is read as positive.
As opposed to the TB skin test, the TB blood test is more convenient as it only requires one visit to a laboratory. During the lab visit, a simple, nearly painless blood draw is performed and the specimen is processed by the lab using specialized equipment. In contrast to the TB skin test, a second visit is not required for a TB blood test. And, because the QuantiFERON test uses sophisticated lab machinery instead of relying on manual skin measurements by a medical provider, this TB blood test is more accurate, particularly in borderline cases.
Question 5 What Are Conversion And Reversion
Conversion: With respect to TB antigens, conversion is the point at which IFN- becomes detectable above the established threshold or cutoff from a previous negative or unknown result.
Reversion: The opposite of conversiona result that goes from above the established cutoff to below the cutoff . While there are no data derived using the QFT-Plus test, there are numerous publications on conversion/reversion based on the QuantiFERON-TB Gold test.
In one study of the QuantiFERON-TB Gold assay, among individuals with quantitative IFN- values just above or just below the cutoff threshold, reversions and conversions were common.5 Per the package insert, it is prudent to confirm a positive QFT-Plus test by the same or alternate method before starting therapy if M tuberculosis complex infection is not suspected.

Quantiferon Tb Gold Test Overview
The QuantiFERON TB Gold In-Tube test is an indirect test for the M. tuberculosis infection which is based upon the measurement of the cell-mediated immune reaction to antigens simulating the mycobacterial proteins CFP-10, ESAT-6, and TB7.7. This combination of three mycobacterial proteins stimulates the patients T-cells in-vitro to release interferon-gamma, which then is measured using ELISA technology. This test detects infections produced by the Mycobacterium tuberculosis complex . The BCG strains and many other non-tuberculosis mycobacteria do not harbor ESAT-6, TB 7.7 and CFP-10 proteins therefore, persons either vaccinated with BCG or even infected with most environmental mycobacteria ought to test negative.
The QuantiFERON TB Gold In-Tube test is done by simply taking a small blood sample from the patient. To do this, a nurse will sterilize the arm, insert a needle to extract a vial of blood, then bandage the arm to ensure that there is no bleeding. The actual process of taking blood should only take about five minutes.
The test results can take seven to 10 days.

Quantiferon Tb Blood Test Results
Conservatively, QuantiFERON TB blood test results typically take approximately six business days to be reported. For many people in busy metropolitan areas, TB blood test results can be available in as soon as three business days. A negative QuantiFERON lab report, when accurate, is the best result. It indicates that evidence of tuberculosis exposure cannot be found in the blood specimen. A negative lab test is similar to a negative TB skin test and, in most cases, does not require a chest x-ray for confirmation or other diagnostic studies.
What does a TB blood test result mean? Positive or indeterminate QuantiFERON TB blood test results typically mean that you will need to get a chest x-ray to confirm that you do not have active tuberculosis. Negative QuantiFERON Gold results imply that you do not have latent tuberculosis at the time the blood was drawn.
Quantiferon Gold Test Cost With Insurance
Most of the health insurance policies in the U.S. cover the cost of TB gold blood test.
